Tutoring/Open Studio Sign Up:

Please note, this form is to be filled out ANYTIME you choose to come into open studio daily from 8:05-9:00am or on THURSDAY after school from 4:30-5:30. You must schedule your own transportation as well as have permission to stay. I require 1 day in advance this form to be filled out for scheduling purposes after school.

I understand this is a space in order to work on additional project and if needed Mrs. Ellington is here to help.
