Please note, this form is to be filled out ANYTIME you choose to come into open studio daily from 8:05-9:00am or on THURSDAY after school from 4:30-5:30. You must schedule your own transportation as well as have permission to stay. I require 1 day in advance this form to be filled out for scheduling purposes after school.

I agree and understand that I must be present starting at 4:30. If I am late and do not notify Mrs. Ellington will leave for the day. I will arrange my own ride and will fill out this form 1 day in advance for Mrs. Ellington.

I have permission from my parent/guardian to stay after school in order to make up or receive additional help on a project. I also agree to give Mrs. Ellington 1 day notice by completing this form for attending open studio.
